This measure assesses the percent of eligible patients with an active diagnosis of hypertension whose most recent blood pressure (BP) recording was less than or equal to 140/90.


Hypertension: percent of eligible patients with an active diagnosis of hypertension whose most recent blood pressure (BP) reading was BP greater than or equal to 160/100 or not recorded (NEXUS clinics cohort).
This measure assesses the percent of eligible patients with an active diagnosis of hypertension whose most recent blood pressure (BP) recording was greater than or equal to 160/100 or not recorded.

Hypertension: percentage of adult persons with diagnosed hypertension whose blood pressure (BP) is adequately controlled (BP less than or equal to 140/90 mm Hg) during the measurement year.
This intermediate outcome measure assesses if blood pressure was controlled among adult persons with diagnosed hypertension.
